The Cancer Center Survivorship Program in the UI Health Mile Square Federally Qualified Health Center (FQHC) provides patients with the tools and resources needed to help continue living a full life during and after cancer care. Survivorship care starts at the time of a cancer diagnosis. Any adult living with a cancer diagnosis can receive survivorship care. The goal is to make sure all of a person’s health care needs are being met and to coordinate care between their primary care provider, oncologist and other specialists.
